Why We Ask What We Ask
Our intake forms are more detailed than what you might fill out at a typical doctor's office, and there is a reason for that. As an upper cervical chiropractor in Lutz, FL, the information we gather before your visit helps us hit the ground running. We are not just asking about your current pain. We want to know about your sleep, your stress levels, past injuries, surgeries, and family health history.
The nervous system does not operate in isolation. What happened to you five years ago in a car accident, or a fall you had as a kid, can absolutely show up in your spine today. The more complete your intake, the better your first consultation will be.
If you would rather fill everything out when you arrive, that is fine too. Just plan to come in about 15 minutes early so you are not rushing before your appointment.
What to Bring
Checklist for Your First Visit
Completed intake forms (or arrive 15 minutes early to fill them out)
Photo ID and insurance card if you are using insurance
Any prior imaging: X-rays, MRIs, or CT scans
Records from previous chiropractors or physical therapists if available
A list of any current medications and supplements
Any notes about when your symptoms started or what triggers them
Insurance question? We work with most major plans. Call us at (813) 578-5889 before your visit and we can verify your coverage so there are no surprises when you arrive.
